This number is projected to reach USD 5.3 billion by 2030.

However, podiatry revenue cycle management (RCM) errors remain a serious concern for practices and billers alike. Next, the claim moves to authorization tracking, and the team ensures all necessary clinical documentation is available.

The billing team then begins coding and charge capture. Once all documentation requirements are met, the team submits the claim, and payment is posted after the payer’s adjudication. Billers may still have to perform accounts receivable (A/R) follow-up, and lastly, patient collections can be reconciled.

What Makes a Podiatry RCM Complex?

Podiatry services involve multiple services, such as routine and surgical foot care. The services are billed with codes that may have several layers, complex documentation, complicated coverage rules, and payer-specific billing requirements. One mistake in any of these areas can render the podiatry RCM process ineffective.

Why is Podiatry Revenue Cycle Management So Important?

The podiatry revenue cycle determines the practice’s financial health and its reputation. A practice with a struggling podiatry RCM faces operational troubles, which also impact the quality of patient care.

In addition, the RCM follows strict guidelines, without which no claims can be accepted. For starters, the diagnosis, procedure, and related procedure codes should correspond. Medical necessity should be proven with proper documentation. Similarly, the podiatry claim should include appropriate modifiers where applicable. Violating any of these above-mentioned requirements can cause the claim to be rejected or denied.

Complex Service Mix

As mentioned earlier, podiatrists operate across a complex mix of services, which makes podiatry RCM more complex. Each service has its dedicated billing compliance and documentation requirements.

These services may include:

Diabetic foot care

Nail and callus debridement

Wound care

Ingrown toenail procedures (avulsion, matrixectomy)

Foot and ankle surgery (bunionectomy, hammertoe correction, neuroma excision)

Fracture or injury treatment

Coding Logic

Each category follows its coding logic and can be handled by an efficient podiatry RCM team. Here are some general rules that apply to podiatry billing:

CPT code selection must match the exact procedure. For instance, CPT 11055–11057 report paring or cutting of benign hyperkeratotic skin lesions based on the number of lesions treated. Similarly, CPT 11719–11721 cover nail trimming or nail debridement services with distinctions based on the service performed and, for applicable codes, the number of nails treated. Billers must review the podiatry CPT codes 2026, as these codes are revisited annually and may be revised, leading to claim errors if used incorrectly.

HCPCS Level II code G0127 is used for trimming of dystrophic nails and may be covered under Medicare’s routine foot care exceptions when applicable coverage and documentation requirements are met.

ICD-10-CM coding must accurately report the applicable systemic condition and documented foot findings supporting the service. If the diagnosis and procedure mismatch, it might become a common reason for routine foot care claim denials.

Modifiers may also be used, particularly the Q7/Q8/Q9 class-finding modifiers. These modifiers identify qualifying class findings for Medicare’s presumption of coverage for certain routine foot care services.

Global Surgical Periods

Applicable surgical CPT codes may have 0-, 10-, or 90-day global periods, depending on the procedure. During an applicable global period, certain related postoperative services are included in the global surgical payment and generally cannot be separately reported unless an applicable exception applies. These may include related services:

Evaluation and management (E/M) visits

Dressing changes

Suture removal

Additionally, global periods require a dedicated modifier. For instance, billing an E/M service during a global period without the appropriate modifier (when the service is separately reportable) may result in a denial.

Documentation Requirements

Podiatry revenue cycle management requires precise and valid documentation for successful claims. According to the Centers for Medicare & Medicaid Services (CMS), insufficient documentation was the reason for 76.4% of improper payments given to podiatric providers during 2024.

While exact documentation requirements may be case-specific, general documentation remains the same, including:

The patient’s systemic conditions (vascular or neurological findings)

Clinical findings specific to the supporting medical necessity

Treatment or services provided to the patient

Class findings and coverage requirements for routine foot care

In addition to these documentation requirements, billing staff have to cross-check eligibility and authorization details before services are rendered. These recommendations may sound basic, but when adopted, practices face fewer podiatry RCM issues.

Podiatry Billing Rules & Payer Guidelines

Efficient podiatry revenue cycle management relies heavily on appropriate and payer-specific billing rules. Unfortunately, these guidelines are updated regularly (quarterly, annually, or at the payer’s discretion), leading to errors. The following billing rules and payer guidelines may apply:

Routine Foot Care & ‘Q’ Modifiers

Routine foot care and podiatry services often intersect. These may include:

Nail trimming

Callus

Corn paring

These routine services are statutorily excluded from Medicare coverage. However, they may be covered if the patient has a qualifying systemic condition backed by a documented “class finding.”

According to the CMS guidance, these findings are segmented into three tiers by Medicare:

Class A: A severe, single finding (e.g., nontraumatic amputation of the foot or a portion).

Class B: Absent posterior tibial pulse; absent dorsalis pedis pulse; or three documented advanced trophic changes.

Class C: Claudication, temperature changes, edema, paresthesias, or burning.

Similarly, for a smooth podiatry RCM, claims should include the corresponding modifiers:

Modifier

Criteria

Q7

1 documented Class A finding.

Q8

2 documented Class B findings.

Q9

1 documented Class B + 2 documented Class C findings.

Important Billing Note: The medical record must support the applicable findings for the service being billed. Thus, billers must verify the latest requirements before moving forward.

Medical Necessity, LCDs, and Prior Authorization

Local Coverage Determinations (LCDs) may establish Medicare coverage and medical-necessity requirements for specific services within a Medicare Administrative Contractor (MAC)’s jurisdiction. These include:

Diagnosis codes

Class findings

Frequency limits justifying coverage for routine foot care

Debridement

Orthotics

Notably, the LCD guidance may vary by MAC jurisdiction. Thus, practices should verify the LCD and associated billing article applicable to their jurisdiction. Similarly, prior authorization is an integral, yet frequently overlooked, billing essential.

Prior authorization or referrals may be required for:

Surgical procedures

Diagnostic imaging

Custom orthotics

DME items

Ideally, podiatry practices must verify requirements during patient intake, not after claim submission. This provides practices leverage in podiatry RCM, but commercial payers follow their medical-necessity criteria in addition to Medicare’s framework.

So, services covered under a plan’s policy may require additional documentation for another plan.

Key Podiatry Revenue Cycle Management Challenges

Podiatry revenue cycle management poses several unique challenges that most practices do not share. For instance, each claim must include an applicable CPT/HCPCS code, with a lesion or nail count (where applicable). If billers miss the Q, laterality, or digit modifier, the reimbursement is delayed.

Coverage status, payer IDs, and billed service category can also cause podiatry claims rejections. In other cases, billers may miss necessary class findings or wound measurements for debridement, which negatively impact reimbursement.

Even if those details are present, misinterpreting exam findings related to the service can be a violation. Proper authorization tied to the CPT code and date range is also a must and often overlooked. If the diagnoses do not support the billed procedure, the claim will be denied for medical necessity.

Procedure add-ons, such as supplies and injections, must be captured, or the practice loses valuable reimbursement. If follow-up services are billed incorrectly, they can cause claim denials or trigger audits against the practice (if recurring).

Best Practices to Improve Podiatry Practice Revenue

Billing and collection errors arise frequently when podiatry RCM processes are subpar. Fortunately, adopting the right practices can significantly boost podiatry practice revenue. The billing staff should:

Track key RCM metrics.

Code documented services only.

Build class findings, per-foot details, and systemic conditions in each claim related to a foot care visit.

Regularly review applicable payer policies and LCDs.
