Besides, it is not on the main campus.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Relocation of an Existing Department<\/strong><\/h3>\n\n\n\n<p>Assume a hospital-based oncology clinic was grandfathered (excepted) because it had been operating since 2010.&nbsp;<\/p>\n\n\n\n<p>However, in 2023, the hospital moved the clinic to a new, larger building 12 miles away to cater to increased patient volume.&nbsp;<\/p>\n\n\n\n<p>Since CMS considers a relocated department to lose its grandfathered status, you must bill chemotherapy administration services with the PN modifier.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Acquisition of a Private Physician Practice<\/strong><\/h3>\n\n\n\n<p>Imagine a hospital that purchased a private cardiology practice in 2024. The hospital then converted it into a hospital outpatient department.&nbsp;<\/p>\n\n\n\n<p>Note that this new department is six miles (in a different zip code) from the hospital and has never billed a service as a hospital outpatient department before 2015. Thus, all facility-level services rendered at this newly acquired site, including echocardiography, must be reported with the PN modifier.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>PN Modifier<\/strong><strong> \u2013 Billing Guidelines<\/strong><\/h2>\n\n\n\n<p>The following are the essential billing guidelines for accurate usage of modifier PN:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Understand When You Should Use the <\/strong><strong>PN Modifier<\/strong><\/h3>\n\n\n\n<p>Discussed below is the key criterion where this modifier applies:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>It applies to all off-campus provider-based departments (PBD) that started billing Medicare on or after November 2, 2015.<\/li>\n\n\n\n<li>Use it if a grandfathered (excepted) department moves to a new physical location. That is, once moved, the department loses its \u2018excepted\u2019 status. Thus, the PN modifier must be appended.<\/li>\n\n\n\n<li>There may be scenarios where an excepted location adds a newly completed category of service. For instance, an existing imaging-only center began oncology services after the 2015 cutoff.\u00a0<\/li>\n\n\n\n<li>When a hospital acquires a private practice after November 2015 and converts it into a PBD.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Know When NOT to Append Modifier PN<\/strong><\/h3>\n\n\n\n<p>Now that you know where to append the PN modifier, it is time to understand when to avoid using it. So, here we go:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Any facility located within <a href=\"https:\/\/www.cms.gov\/files\/document\/mln006764-evaluation-management-services.pdf\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">250 yards<\/a> of the main hospital&#8217;s buildings is considered an on-campus department, i.e., you cannot use modifier PN.<\/li>\n\n\n\n<li>Locations that were already billing as hospital departments before November 2, 2015, and have not moved or changed status, cannot bill with the PN modifier. Use modifier PO instead.<\/li>\n\n\n\n<li>Services provided in a dedicated hospital emergency department are exempt from these site-neutral payment reductions.<\/li>\n\n\n\n<li>Rural Health Clinics and Federally Qualified Health Centers (FQHCs) have different payment structures. Thus, they cannot report services with the PN\/PO modifiers.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Fulfill Documentation Requirements<\/strong><\/h3>\n\n\n\n<p>Your documentation must support the use of the PN modifier.
